NCR / CAR
Non-conformance reports and the corrective actions that close them — raising an NCR, adding a CAR, resolving it, and the pick-lists behind them.
Compliance → NCR / CAR (/operations/compliance/ncr) records things that went wrong and what was done about them. It replaces iDrv4’s QAR page.
- An NCR (non-conformance report) is the problem: a chain-of-responsibility breach, a customer complaint, an incident. One NCR can name several vehicles and several drivers.
- A CAR (corrective action) is the fix: a short-term fix, a long-term fix, who is responsible and when it is due. An NCR can have several.
Incidents are not a separate page. Record one as an NCR with the classification Incident.
The page
Section titled “The page”The tiles count Open NCRs, Open corrective actions and CARs past due (an open CAR whose due date has passed).
The list opens on Open (pending, processing). Change Status to All or to one status, or pick a Vehicle, and click Apply. Each row shows the Control no, the Raised date and who raised it, the Classification and breach category, the Vehicles / drivers, the start of the Problem, how many CARs it has (with an open count) and its Status. A row marked iDrv4 was imported.
Export CSV downloads the list as filtered.
The vehicle page has an Infringements & NCR tab listing the NCRs that name that vehicle. Its Open in NCR / CAR link opens this page filtered to that vehicle.
Raise an NCR
Section titled “Raise an NCR”- Click Raise NCR. A drawer opens on the right.
- Raised defaults to today. Problem details is required: describe what went wrong.
- Fill in what applies:
- Raised by and Status.
- Classification, Breach category and Code, from the lists.
- Breach areas: for General, Fatigue and Mass, choose a severity (Minor, Substantial, Severe, Critical or Major) or leave it Not involved.
- Vehicles and Drivers: Ctrl/⌘-click to pick several.
- Customer and Run / manifest ref (free text).
- Distribution (Management, Admin, Operations, Driver, Others) and Initial action (Monitor, Investigate, Risk assessment, No action).
- Person involved and the date they Signed.
- Approved by / Approved and Closed by / Closed.
- Notes and Supporting documents.
- Click Save NCR.
Onyx gives it a control number (for example NCR-00001), shows it as the drawer title, and the Corrective actions (CAR) section appears below the form. The list refreshes when you close the drawer.
To open an existing NCR, click the folder button at the end of its row.
Add a corrective action
Section titled “Add a corrective action”In an open NCR, click Add CAR and fill in:
- Root cause.
- Short-term fix and/or Long-term fix. At least one is required.
- For each fix, the date it was closed out and by whom. A close-out date needs both the fix and the name.
- Responsible, Due and Status (Open, Closed or Cancelled).
- Approved by and Approved.
- Evidence files.
Click Save CAR. Each CAR is listed with its fixes, a ✓ and date on each fix that is closed out, and who is responsible. Click Edit to change it.
Adding a CAR to a Pending NCR moves it to Processing.
Statuses and the rules
Section titled “Statuses and the rules”An NCR is Pending, Processing, Resolved, Terminated or Cancelled. Pending and Processing count as open.
Onyx refuses:
- Resolving an NCR while a CAR is still open. Close or cancel the CAR first. A cancelled CAR does not hold it open.
- Closing a CAR while a fix it describes is not closed out. Enter the close-out date and who for each fix first.
- Resolved or Terminated without a Closed date. The Closed date cannot be before Raised.
- Raising a new NCR as Resolved or Terminated. A new NCR starts open.
- A new CAR on a closed NCR. Set the NCR back to Pending or Processing first. Add CAR is hidden on a closed NCR.
NCR lists (admins)
Section titled “NCR lists (admins)”Lists opens the pick-lists: Breach categories, Codes, Classifications and CAR people. CAR people are the names suggested for Approved by, Closed by and Responsible. You can still type another name.
Type a name and click Add. Click Turn off to stop offering one. Entries are never deleted. An NCR that already uses an entry keeps it, shown as (no longer offered).
Onyx starts with one classification, Incident. Add the rest your business uses.
Who can change things
Section titled “Who can change things”Operations and Admin users can raise and edit NCRs and CARs. Only Admin sees Lists. Other operations-portal users can open NCRs read-only.
⚠️ If the page says NCR / CAR is being set up, the database update for this feature has not been applied yet on this environment.
